Tesamorelin is a synthetic polypeptide composed of 44 amino acids, analogous to growth hormone-releasing hormone (GHRH) with specific modifications at both the N-terminus (acetyl group) and C-terminus (trans-3-hexenoic acid group) to improve stability and resistance to enzymatic degradation.

Tesamorelin interacts with GHRH receptors on pituitary somatotroph cells, initiating a signaling cascade that enhances cyclic adenosine monophosphate (cAMP) production through adenylate cyclase stimulation.
